* ALL ADULTS, this includes Chaperones, Parents, Youth Ministers, Guardians and Volunteers, attending the ABLAZE 2024 Retreat must fulfill and be in CURRENT compliance with their Diocesan policies and requirements for providing a safe and secure environment for minors by completing any Diocesan required training and a current criminal background check. See SAFE ENVIRONMENT section of the this web site, under Registration.
* All Adults must complete 3 Forms PRIOR TO the Retreat: (1) the Adult Liability Release and Registration Form (Form B - both sides), (2) the Adult Travel Form and (3) the Adult Code of Conduct. Those chaperones arriving at the ABLAZE 2024 Retreat who have not submitted the required documentation or whose names cannot be verified as being in current compliance in the Safe Environment database of the Diocese of Dallas will NOT be admitted to the Retreat. NO EXCEPTIONS.
* Chaperones are adults age 21 and over who arrive with, attend with, stay with and are responsible for their youth during the Retreat.
* Parent/guardian assumes the responsibility for finding a chaperone. Parents are welcome as chaperones for their children. Youth/young people are anyone attending the Retreat who is 17 or younger. Each youth must have a designated chaperone named on their Registration Form.
* There must be at least TWO adult chaperones with every group of 1 up to 14 young people. If you have more than 14 youth in your group, you must have at least one additional adult chaperone for every additional one to seven youth in your group. (A group of 1 to 14 youth needs at least 2 chaperones; a group of 15 to 21 youth needs at least 3 chaperones; a group of 22 to 28 youth needs at least 4 chaperones, etc.).
* Members of the Diocese of Dallas: You must be fully compliant and CURRENT with the SAFE ENVIRONMENT certification process required by the Diocese of Dallas, including completion of all Diocesan required steps of the Safe Environment Screening procedure. It is your responsibility to be current with the annual renewal process. Upon receiving your Adult Registration Form and Liability Release (Form B), including the month/date of your birth, and the Safe Environment number provided by the Diocese of Dallas to you (and/or your parish), the Diocese of Dallas will confirm that your name appears in the Safe Environment database of the Diocese of Dallas as current and fully compliant. Adults not listed as current in the database will not be admitted to the Retreat. No exceptions.
* Members of other diocese: Attach a LETTER to your Adult Registration Form and Liability Release (Form B), from your parish or diocese stating that you are in FULL AND CURRENT compliance with the Zero Tolerance Policy of the USCCB, as implemented by your Diocese, and that you have completed the required Diocesan training, including the date of training, and have cleared a current criminal background check through your parish/Diocese, including the date the most recent background check was conducted.
* Registration Fee: $30 per adult/chaperone. This fee helps offset the basic expenses of the Retreat (food, beverages, facilities costs, etc.).
ABLAZE does not provide medications. Chaperones, please check that participants bring any medications and supplies, such as inhalers, that they might need.
Any participant who is a minor, age 17 and younger, MUST HAVE:
1. a designated chaperone
2. written permission signed by a parent/legal guardian if planning to leave the ABLAZE Retreat during Retreat hours. The Chaperone must accompany minor to the sign-out monitor, and parent must come inside to sign out your child.
